Insurance

Payment for services is required at the time of service; however, many medical plans can be billed directly by us. Please discuss this with the receptionist.

Copays, Co-insurance, and Deductibles

All co-payments must be paid on the date of service. Failure to do so will result in a $20.00 surcharge. All co-insurance and deductibles are within 14 days of receiving an EOB (explanation of benefits) and statement from our billing department.

Cash Patients

Cash patients must pay in full on the date of service and they will receive a discount according to the doctor's discretion.

We accept Visa, Mastercard, American Express, personal checks, and cash. There is a $35.00 charge for checks returned by the bank.

PPO Insurance

Please be aware if your policy has a deductible and/or coinsurance. Prepare to make payment upon receiving a statment from our office and Explanation of Benefits from your insurance.

These charges may be in addition to a copayment.
